glycemic variability: clinical implications

bedtime-morning glucose differential ≥50 mg/dL), hypoglycemia (aware or unaware), and high variability. The proportion of T1DM patients who could achieve optimal glycemic control increased slightly from 14.3 to 18.2% but the proportion of T2DM patients who could achieve optimal glycemic control increased from 30.8 to 53.8%. A1C was the measurement studied in clinical trials demonstrating the benefits of improved glycemic control on microvascular and macrovascular outcomes (figure 1 and figure 2) . "Excursion" simply means fluctuations in your blood sugar, but the term is usually used when talking about how much the blood sugar numbers change with meals. In diabetic patients, an excursion is a rapid change in blood sugar, usually referring to a rapid upwards spike in blood sugar. Emma S Scott, Andrzej S Januszewski, Rachel O’Connell, Gregory Fulcher, Russell Scott, Antero Kesaniemi, Linda Wu, Stephen Colagiuri, Anthony Keech, Alicia J Jenkins, FIELD study investigators, Long-Term Glycemic Variability and Vascular Complications in Type 2 Diabetes: Post Hoc Analysis of the FIELD Study, The Journal of Clinical Endocrinology & Metabolism, Volume 105, Issue … (lasting minutes or hours), represent an independent risk factor. Navigating through these challenges requires an understanding of the relationship between hyperglycemia, glycemic variability, and risk, as well as how to combine … Recent clinical data indicate that GV is associated with increased risk of hypoglycemia, microvascular and macrovascular complications, and mortality in patients with diabetes, independently of glycated hemoglobin level. Distribution of HbA(1c) levels for children and young adults in the U.S.: Clinical Journal of Sport Medicine 18.5 (2008): 423-431. The purpose of the current study was to determine the role of mastication on … In recent years, GV has become increasingly clinically relevant, because of a better understanding of the need to reach target A1C while avoiding hypoglycemia. Glycaemic variability is an integral component of glucose homoeostasis. Glucose fluctuations in subjects with normal glucose tolerance, impaired glucose regulation and newly diagnosed type 2 diabetes mellitus. Diabetologia 1985;28:119-21. 1 author. This effect increases the variability of glycemic responses in fats consumed in normal meals are usually smaller than those IDDM subjects and provides a rationale for stabilizing blood required to obtain effects in experimental situations (25). Course Description: Join diabetes experts Anne Peters, MD, and Jay Shubrook, DO, as they discuss measures other than HbA1c to evaluate the clinical outcomes of people with type 1 diabetes (T1D) and explore ways to help address common patient concerns, such as hypoglycemia, glucose variability, and diabetes distress. Clinical Endocrinology, 76(6), 810–815. Glycemic variability measures at baseline, 8- and 16-week endpoints were compared between groups. Intraday glycemic variability is also suggested to accelerate coronary artery disease in high-risk patients. Glycemic variability: Clinical implications. However, A1C does not take into account fluctuations in blood glucose levels known as glycemic variability (GV). to Improve Diabetes Management. However, the relationship between GV and BRS has not been clarified. Glycemic variability (GV) refers to fluctuations in blood glucose levels. Indian J Endocrinol Metab 2013;17:611–619. Results: At the eight-week endpoint, between-group differences demonstrated significant reduction in several GV measures with rtCGM compared to iscCGM (GRADE%hypoglycemia, index of glycemic control [IGC], and average daily risk range [ADRR]; P < .05). Crit Care Med. Long-term glycemic control is a well-known risk factor for diabetic chronic complications [], but there is recent evidence that glucose variability (GV) may also play a role in their development [].GV can be interpreted as intraday or interday glucose variation or long-term variation of HbA1c, which can be assessed by standard deviation (HbA1c-SD) and coefficient of variation (HbA1c-CV) []. A1C does not provide a measure of glycemic variability or hypoglycemia. However, the significance of HbA1c variability is unknown at present. GLYCEMIC VARIABILITY. Glycemic variability: Clinical implications. However, the implications of CGMS in routine clinical practice are limited due to its high cost and complexity. Hirst JA, McLellan JH, Price CP, et al. Safe, effective tight glycemic control (TGC) of critically ill patients can improve outcomes [1–4], but it is difficult to achieve consistently [5–7].Glycemic level and variability in TGC are a function of variability in insulin sensitivity, potentially resulting from the level and evolution of the stress response [], and are independently associated with mortality [9–12]. Despite a lack of randomized controlled trials, recent clinical data suggest that long-term glycemic variability, as determined by variability in hemoglobin A1c, may contribute to the development of microvascular complications. AbstractGlycemic control remains an elusive goal for most patients with type 2 diabetes. Interpretation of continuous glucose monitoring data: Glycemic variability and quality of glycemic control. Jones AG, Hattersley AT. This evidence highlights the potential complications associated with pharmacotherapy for improving cognition by targeting glycemic control. A literature search was carried out satisfying the predefined inclusion criteria from Pubmed, Embase, and Cochrane Library. There is clinical data showing that people with high blood glucose variation are at a higher risk of having hypoglycemia or even severe hypoglycemia than those with low glycemic variation. Part of the controversy is due to methodologic variables that can markedly affect the interpretation of glycemic responses and the GI values obtained. The glycemic index (GI) is a tool to measure how individual foods are expected to impact blood-glucose levels. A food is scored on a scale of 0 to 100 according to how much it raises blood-glucose levels as compared with blood-glucose levels after the consumption of 100 grams of glucose. The poor-controlled blood glucose (BG) significantly increases the risk of DM-related vascular complications including coronary artery disease (CAD), stroke, kidney disease, and so on. Introduction In April 2017, the Roundtable on Obesity Solutions of the National Academies of Sciences, Engineering, and Medicine held a workshop titled The Challenge of Treating Obesity and Overweight with the objective of exploring what is known about current obesity treatment approaches in adults and children and the challenges in implementing them. Glucose variability is associated with of insulin sensitivity during the first 4 days of critical ill- intensive care unit mortality. However, glycemic variability (GV; the amplitude, frequency, and duration of glycemic fluctuations around mean blood glucose) is an emerging target for blood glucose control. A1C was the measurement studied in clinical trials demonstrating the benefits of improved glycemic control on microvascular and macrovascular outcomes (figure 1 and figure 2) . GLYCEMIC VARIABILITY. 2015;145(4):736-741. Methods . Glycemic variability (GV) is one more tool to explain relation between hyperglycemia and increased cardiovascular risk in diabetic patients. The current study aimed to assess the association of GV with myocardial infarction. Although glycemic control is known to reduce complications associated with diabetes, it is an elusive goal for many patients with diabetes. A growing body of evidence supports GV as an independent risk factor for diabetes complications. In both type 1 and type 2 diabetes, large prospective clinical studies have shown a strong relationship between time-averaged mean levels of glycemia, measured as hemoglobin A 1c (HbA 1c), and diabetic complications. There is no argument that improving mean levels of glycemic control as judged by assays for glycated hemoglobin (HbA1c) reduces the risks of microvascular complications and cardiovascular disease events in patients with type 1 and type 2 diabetes. Introduction Although glycemic control is known to reduce complications associated with diabetes, it is an elusive goal for many patients with diabetes. 11. variability. Glycemic Variability in Patients With Early Type 2 Diabetes: The Impact of Improvement in β-Cell Function. SGLT-2 inhibitors are novel anti-diabetic drugs and currently there are not published studies that evaluated their effects on glycemic variability calculated by the widely used methods like mean amplitude of glycemic excursion (MAGE), standard deviation of the mean glycemia (SD), coefficient of variation (CV) and interquartile range (IQR). Lachin JM, Bebu I, Bergenstal RM, et al. Meeting these goals will have important clinical implications to reduce diabetes complications and improve quality of life in young adults with T1DM. In recent years, GV has become increasingly clinically relevant, because of a better understanding of the need to reach target A1C while avoiding hypoglycemia. Prognostic Implications of Single-Sample Confirmatory Testing for Undiagnosed Diabetes: A Prospective Cohort Study. In this review, the latest data on the impact of glycemic variability on brain function and neuroinflammation will be presented. In both type 1 and type 2 diabetes, large prospective clinical studies have shown a strong relationship between time-averaged mean levels of glycemia, measured as hemoglobin A 1c (HbA 1c), and diabetic complications. 1.1 In patients aged 65 years and older with newly diagnosed diabetes, we advise that an endocrinologist or diabetes care specialist should work with the primary care provider, a multidisciplinary team, and the patient in the development of individualized diabetes treatment goals. Continuous glucose monitoring (CGM) provides comprehensive assessment of daily glucose measurements for patients with diabetes and can reveal high and low blood glucose values that may occur even when a patient’s A1C is adequately controlled. Heart Rate Variability . In this study, we used flash glucose monitoring (FGM) and evaluated glycemic intraday variations, and then examined the association with HbA1c variability. 1.1 We suggest that the diagnosis of polycystic ovary syndrome (PCOS) be made if two of the three following criteria are met: androgen excess, ovulatory dysfunction, or polycystic ovaries (PCO) (Tables 1 and 2), whereas disorders that mimic the clinical features of PCOS are excluded.. The objective of this study was to identify factors associated with sustained poor glycemic control, some glycemic variability, and wide glycemic variability among diabetes patients over 3 years. Questions concerning glucose targets that have emerged from recent outcomes studies further complicate glucose control strategies. Women with type 1 and type 2 diabetes should ideally have ophthalmological assessments before conception, during the first trimester, as needed during pregnancy, and within the first year postpartum .The risk of progression of retinopathy is increased with poor glycemic control during pregnancy, and progression may occur for up to 1 year postpartum . Author information. Poor outcomes in patients with diabetes and COVID-19 are increased more than 4-fold compared with patients without diabetes. Another important consequence of large intraindividual glucose variability is that the patient has to perform SMBG more frequently, which is a burden for most diabetes patients both from a psychological and a financial point of view. Although glycemic variability is more difficult to measure than HbA1c, future trials investigating the effects of glycemic control on brain health should attempt to include it as a measure. 1 Introduction. 3, Hagerstown, MD 21742; phone 800-638-3030; fax 301-223-2400.
